  1. Click here to download a claim form, print and complete the form – some information you’ll need will be on the back of your EyeMed ID card.

  2. Mail your completed claim form and itemized receipt from Optical Shop of Westport to:
    First American Administrators
    ATTN: OON Claim Department

    Mason, OH 45040-711

EyeMed asks to allow at least 14 calendar days to process your claim. A check and/or an explanation of benefits will be mailed to you within a week from the date your claim is processed.
